The Care3SM Wellness program is a comprehensive nursing, therapy and wellness system that integrates both proactive and reactive services to help Brookdale residents enhance their health and fitness and retain or improve their independence.
There are three distinct segments of the Care3 Wellness Program: Rehabilitation, Fitness and Education. Each segment is comprised of multiple components, activities and processes. Primarily proactive in nature, our approach is a holistic one that is pre-emptive where possible and treats underlying causes as well as symptoms. Comprehensive nursing, rehabilitation, fitness and education services include Medicare certified Home Health nursing/therapy, Outpatient physical, occupational and speech therapy services at the majority of Brookdale locations.
Innovative Senior Care professionals assess each resident on a voluntary basis and refer them to the appropriate segments of the Care3 Wellness Program based on their personal level of health, fitness, or mobility. The residual benefit for residents of Brookdale Senior Living communities is that those who engage in Care3 programs at every level have the potential for better health at any age in any senior living environment, regardless of their particular abilities.
The Rehabilitation Care segment consists of physical, occupational and speech therapy, as well as skilled Home Health nursing and therapy services (at select locations). With the goals of helping residents to live an active lifestyle at the greatest levels of independence possible, Innovative Senior Care therapists and nurses work to meet the individual needs of each Brookdale Senior Living resident. With professional services from Innovative Senior Care, medications can be managed, health conditions can be corrected, chronic pain can be managed, incontinence can be improved and falls can be prevented, in addition to a variety of other specialized services.
The Fitness Care segment provides individualized exercise and wellness programs with components designed to improve strength and balance making active adult living not just possible, but optimal at any level of ability.
Through the Care3 Education segment, Brookdale Senior Living residents have access to information needed to facilitate healthy aging concepts inherent to their age, level of ability or disability and lifestyle. With a goal of promoting improved health and general wellness, Care3 Educational activities address issues that seniors face daily through support group programs and other health-based activities.
